NDIS Transition Support Worker

2x NDIS Transition Support Worker Positions
Contracts until end of Feb 2020.

The Transition Program provides psychosocial support services to participants while supporting and assisting participants to test their eligibility for the National Disability Insurance Scheme (NDIS), or transition to appropriate support under the Continuity of Support (CoS) program. The Transition Program is only for participants that were previous participants in one of the three Community Mental Health Programs: Partners in Recovery (PIR), Personal Helpers and Mentors (PHaMs) or Day to Day Living (D2DL).

To be successful for this position you will need:
Qualifications and/or extensive experience in mental health,
Prior experience and knowledge of the NDIS and its service models,
Have excellent interpersonal and written communication skills.
Ability to work independently and as part of a team.
